Warts
Cryotherapy
We are pleased to offer cryotherapy once again for small skin problems such as warts, skin tags, and other superficial lesions. We have purchased a cryopen for this which employs a cannister of nitrous oxide, rather than the usual liquid nitrogen. It's not as uncomfortable and can be used in a more precise manner for difficult to reach areas. In keeping with our philosophy of trying to keep charges reasonable for cash patients, we charge $130 for the destruction of benign lesions up to #14 (most practices in our region charge insurances approx $200 for the same CPT code, 17110) and $125 for destruction of skin tags up to #15 (most offices would charge insurances about $240 for the same, according to https://www.fairhealthconsumer.org ).
Medical Weight Loss
I'm still nowhere's near my normal strength and endurance, but I'm working out with light weights at home and going up/down stairs at the beach with a friend. Aside from the muscular atrophy associated with the dramatic weight loss, the orthostatic side effects (dizziness) from my therapies have also made me a significant fall risk and I'd taken several good tumbles earlier this month resulting in some emergency department visits. Fortunately, nothing broken or bleeding yet (had to make sure b/c I'm also on a blood thinner to dissolve a cancer related thrombus in a mesenteric vessel).
Primary Care
What about holistic medicine? Do M.D.'s practice holistic medicine? As I noted, the concepts of structure and function being inter-related, etc. have all been pretty much accepted by the medical community nowadays, but whether or not most M.D.'s actually think about this is another matter. Philosophically, Osteopathy can be considered more closely aligned historically with Chinese medicine, which took a holistic approach. Allopaths (M.D.'s) philosophy stems way back to ancient Egypt where they used to have individual gods for different things. Body parts were sort of compartmentalized - there may have been a god to care for extremities, another for chest, etc.. This type of thinking does not lend itself as well to a holistic approach. The result today: the majority of D.O.'s practice primary care medicine and the majority of M.D.'s specialize.
Family Practice
Raised in New Jersey, Dr. Pearson graduated from Gettysburg College, in Pennsylvania, and received his Osteopathic medical degree from the Kirksville College of Osteopathic Medicine, in Missouri, in 1983. He returned to the NYC metropolitan region to complete a rotating internship and residency program. Dr. Pearson earned his board certification in 1986 from the American Osteopathic Board of Family Practice and practiced in northern New Jersey until moving to San Diego in 1988.

MRI
Quite simply, a good doc does not order a test unless it's going to change his/her treatment. While this might not be such a big deal when ordering cheap tests (do these really exist?), it becomes a significant issue when it comes to the more expensive technologies such as magnetic resonance (MR) scanning (MRI scans may easily exceed $1000).

For most women starting HRT, no special labs need to be ordered as the diagnosis is fairly obvious. If Dr. Pearson is your primary care physician, he might find that you're due for routine labs or other studies (e.g. mammography, DEXA) and might have to order those. If, on the other hand, you're a member of Kaiser Permanente (or some other medical group) and are seeing Dr. Pearson in consultation for HRT only, those studies can be ordered by your primary care physician within that system.
Computed Tomography
There are occasions where the diagnosis of a fracture simply doesn't make any difference in the subsequent treatment. Fractures of the toes and ribs are probably the most common examples of these. All common toe injuries are treated by "buddy taping" the affected toe to the adjacent one, whether a sprain or simple fracture. In the case of ribs, they are composed of bone and cartilage, the latter of which cannot be visualized with routine x-rays. Rib studies are notorious for missing rib fractures. For example, I cared for a patient involved in a motor vehicle accident who complained of left rib and flank pain. Plain films obtained by the emergency room physician were interpreted as being negative for fracture. When I saw him, I was concerned for a possible spleen injury so I ordered a CT scan of the region which revealed the presence of 7 broken/fractured ribs! Regarding their treatment, we do not apply casts to chests for rib fractures as this would compromise breathing (we even try to avoid rib belts, if possible, for this reason). Hence, painful ribs are treated with analgesics ("pain killers"), ice and then heat, etc - whether they are broken or merely contused (bruised). Bottom line: no need for rib x-rays unless we clinically suspect a more severe injury such as a pneumothorax (and then the radiographic study of choice is a chest x-ray looking at the lungs, rather than a rib series).
Chemotherapy
My therapy will be more intense - daily radiation treatments for about 5 weeks and will be complemented by chemotherapy. Our thinking is that this should address any lymphadenopathy that may be present in addition to the primary tumor. With this aggressive course, my body will take more of a beating with the most common adverse effects including fatigue and delightful GI problems (nausea/vomiting and diarrhea). It’s estimated that I’ll need 3-4 weeks after the conclusion of the treatments to feel back to normal. I go in to “get mapped” for radiation next week and will most likely begin this regimen by early May. We’re hopeful that I’ll be over most of the nasty effects by late June and feeling well enough to attend Comic-Con (July), Magic Live (early August), and Burning Man (Late August).
Radiation Therapy
That’s where we’re at now. Laurie and I have met with our team (oncologist, surgeon, and radiation oncologist) and have decided to forego the Whipple surgical procedure at this time (always a difficult thing to try with lots of potential problems) in order to pursue a more aggressive type of radiation therapy known as “chemo-radiation.” While radiation alone can destroy cancer cells effectively, the problem is that it does not address microscopic cancer cells that may be hiding within the region. Radiation alone would normally require only about 5 treatments.
Radiation Oncology
Let's recap, shall we? I diagnosed myself with pancreatic cancer at the very end of April, following a bout of painless jaundice due to biliary tract obstruction. The obstruction was dealt with by placement of a stent and we (Laurie and I) met with a team of excellent docs (specialists in hematology/oncology, Transplant surgery, and radiation oncology) to plan out a treatment strategy. We went with 3 months of chemotherapy with the intent of getting a Whipple Procedure if the tumor burden had been adequately reduced.

Sciatica
I could go on forever on the subject of imaging the acute (fresh/new) low back injury. The fact is that out of 100 back injuries, less than 5 will ever require surgery, including many disc herniations. Plain x-rays cannot visualize muscles, discs, nerves, or ligaments, nor can they identify facet subluxations (the "kinks" that patients commonly present with in the office). MR imaging can identify the majority of disc herniations/protrusions, but we would only order MR examinations if surgery was being considered because, as just noted, the majority of patients with acute disc injuries get better without surgery. The natural course of acute low back pain is well-recognized, as are the treatment options. Early imaging studies do not add anything to the management of the typical case of acute low back pain. Of course, exceptions may exist and there are some "red flags" in the patient's history that we look for in order to identify who might really have an underlying condition warranting further investigation. The National Guideline Clearinghouse (U.S. Government) has published it's guidelines for the evaluation/management of low back pain or sciatica in the primary care setting. (They may be found here.) We have the capability to perform minor office surgical procedures such as biopsies and excisions of many skin lesions, as well as cryosurgery ("freezing" of pre-malignant and other benign skin lesions). We treat most types of common dermatitis and other skin conditions, as well. However, should a patient not observe appropriate improvement with my treatment, I would not hesitate to make a referral to one of our fine local dermatologists. These are but just a few common examples.

There are numerous studies demonstrating the extraordinary amounts of money wasted on unnecessary radiographic examinations. For example, physicians in Ottawa, Canada, looked at the vast numbers of plain x-rays that were ordered for typical injuries to the knee, ankle, and foot. They observed that while most patients who arrive at emergency rooms with an ankle sprain are sent for an X-ray, less than 15% actually have a fracture. These physicians developed the Ottawa Ankle Rules, a set of guidelines that can help doctors avoid ordering unnecessary X-rays simply by observing how patients walk and by feeling their feet. These rules were recently validated, yet again, by a Swiss study that concluded that they "can rule out ankle fractures with almost 100% accuracy and cut down on thousands of needless X-rays every year." As a sports med doc, I've used these rules over the years and have yet to miss a clinically significant fracture.
