But even if you don't smoke, you have an increased risk (about 30 percent) of having or developing another malignancy. It is therefore imortant to keep up your routine screening and examinations with your family physician, including periodic chest xrays, mammograms for women, and colonoscopy.

Urinary Incontinence
No treatment has provided cure rates that are superior to radical prostatectomy. Removal of the disease is a sure cure if the cancer is confined to the prostate, and pathologic examination of the specimen provides important prognostic information. If the disease is confined to the prostate survival is equal to that of men of the same age who do not have cancer. Optimal candidates for radical prostatectomy are healthy men who have a life expectancy of 15 or more years. The advantages of prostatectomy include, in addition to an excellent chance of cure, the ability to know the risk of recurrence based on the pathologic examination of the prostate as well as the follow up PSA. Unlike treatments that leave the prostate in place, PSA after prostatectomy should be zero (or 0.2 or less, which is the lower limit of accuracy of many labs). A rise confirmed above 0.2 permits early treatment of recurrent or residual disease. Radiation therapy can used as salvage if disease recurs in the bed of the prostate. In contrast, surgery is generally not a good option if disease recurs after radiation because of the high risk of complications related to the scarring and poor blood supply after radiation. The disadvantages of prostatectomy include the risks of surgery, including urinary incontinence, which generally resolves with Kegel's exercises, erectile dysfunction, which generally improves if the nerves are spared.
Immunizations
What to expect with BCG: You should not expect to have any serious reaction- these are quite rare and can be avoided by reducing the dose of BCG. With the first and second of the 6 initial weekly BCG instillations most people have no symptoms other than the irritation associated with passing the catheter. Beginning with the second or third instillation you may have some mild burning and increased frequency of urination, and occasionally some bleeding occurs. These symptoms often increase with later treatments, but if they are bothersome the dose of BCG can be reduced. Burning and frequency generally last for 1 to 3 days and may be associated with malaise or mild "flu-like" symptoms typical of many vaccinations. Sometimes nausea may occur, along with low grade fever or a mild chilling sensation. Dr. Lamm will give you a prescription for two antibiotics to take should shaking chills, temperature above 101, or severe, prolonged burning and frequency occur. These antibiotics (usually Cipro or Ofloxacin twice daily and isoniazid once daily) are specific for BCG. Please call before taking the antibiotics, but if you cannot connect, go ahead and take as directed.

Bladder Cancer
Despite the success of BCG immunotherapy for superficial bladder cancer, many patients eventually fail. Because BCG prolongs the time to recurrence and progression, patients who fail BCG and other treatments are often at an age when cystectomy is difficult, and with multiple co-morbid conditions, it can even be contra- indicated. We are therefore searching for improved treatments for bladder cancer.

Kidney Cancer
Kidney cancer occurs in about 30,000 Americans each year and causes death in about 12,000. If caught and treated early, it is commonly curable. More than 100,000 in the US have survived kidney cancer. Unfortunately it appears that the number of people who have kidney cancer is increasing.
Kidney Stones
HIFU (High Intensity Focused Ultrasound): HIFU, like external beam radiation therapy (EBRT), uses external energy that passes through the skin, is focused on the prostate, and kills the cells. Unlike EBRT, which requires daily treatments for a month and a half, HIFU is a day procedure. Under anesthesia an ultrasound probe, similar to the one you had to guide your biopsy, is inserted into the rectum. The prostate is carefully imaged and measured. With a computer controlled robotic arm guided by the surgeon the prostate is destroyed by focusing intense acoustic waves repetitively on small portions of the gland. These focused sound waves work like a scalpel to remove the prostate; heat generated by the waves destroy the gland. These effects can be observed by the surgeon during the procedure as vaporization bubbles in the gland. While this is a surgical procedure that requires special training, it is done like by waves passed through the body, without an incision. It is analogous to shock wave lithotripsy (ESWL) used to break up kidney stones. ESWL revolutionized the treatment of stone disease; HIFU has the potential of doing the same for cancer treatment. Optimal candidates for HIFU are currently those with low to intermediate grade (Gleason 7 or less) prostate cancer confined to a gland that is not greatly enlarged (less than 50 cc in size) who are not candidates for or decline open or robotic surgery and, if they live in the US, are willing/able to travel to another country. The advantage of HIFU is that it is a non-invasive outpatient procedure that destroys the prostate with little risk of erectile dysfunction (ED) or incontinence. From my experience it is clearly the best tolerated procedure: patients walk out of the hospital feeling well. Narcotic pain medicine (codeine) is not required. The main disadvantages of HIFU are the lack of 15 year follow up (though PSA and biopsy results are good), the lack of FDA approval for non-study use in the US, and the need for a supra pubic catheter (a tube inserted directly into the bladder through the lower abdominal wall) for 3 to 4 weeks after the procedure. Like ESWL for stones, where the sand and gravel from the stone must be passed in the urine, with HIFU the destroyed prostate must be passed in the urine. The suprapubic tube provides a safety valve if the urethra becomes clogged with debris.
Testicular Cancer
Some studies have raised questions about a possible relationship between vasectomy (an operation to cut or tie off the two tubes that carry sperm out of the testicles) and the risk of developing cancer, particularly prostate and testicular cancer. Such a relationship, if proven, would be of importance, as about one in six men over the age of 35 in the United States has had a vasectomy.
Colon Cancer
Causes and Risk Factors for Kidney Cancer Smoking significantly increases the risk of kidney cancer (like lung cancer, of course, but also bladder cancer, colon cancer, and others). A family history of kidney cancer also increases the risk, and many associated genes have been identified. Polycystic kidneys, which is generally inherited, as well as renal failure and dialysis, which also can produce cystic kidneys, are associated with kidney cancer. As with prostate cancer and other cancers, diets high in red meat, especially fried meat increase risk for kidney cancer. Renal cysts, however, are common after age 60 and generally are completely benign.
Prostate Cancer
Cancer of the prostate is the most common cancer in men. The risk of prostate cancer increases with age, a family history of prostate cancer, and diets high in animal fat/red meat or calcium, and/or low in fruits and vegetables. Prostate cancer tends to grow quite slowly. Therefore, when it occurs at advanced age it may not require treatment. Lung Cancer
Prostate cancer is the most common non-skin malignancy in men and is responsible for more deaths than any other cancer, except for lung cancer. However, microscopic cancer is found at autopsy in many if not most men. Approximately 16% of American men will be diagnosed with prostate cancer sometime in their life.

Ultrasound
The appearance of prostate tissue in ultrasound images changes when it is frozen. To be sure enough prostate tissue is destroyed without too much damage to nearby tissues, the surgeon carefully watches these images during the procedure. A suprapubic catheter is placed through a skin incision on the abdomen into the bladder so that if the prostate swells after the procedure (which usually occurs), it won't block the passage of urine. The catheter is removed 1 to 2 weeks later.
MRI
Stage I is confined to the prostate and cannot be felt on DRE. Stage II is also confined, but can be felt. Stage III disease is beyond the capsule or lining of the prostate, but not in lymph nodes, while stage IV disease is in lymph nodes or other parts of the body such as bone. Stage is determined by DRE, biopsy, PSA, and if the PSA is above 20 bone scan and other tests such as CT or MRI scan.

Clinical research: Industry has withdrawn funding for our study of TICE BCG plus Intron A interferon and Oncovite high dose vitamins. With 670 patients entered in this randomized trial, Dr. Lamm is seeking further funding to complete and analyze this important work. Ongoing studies of gene therapy, improved chemotherapy and new immunotherapies are also underfunded.
Radiation Therapy
Hormonal therapy targets cancer that has spread beyond the prostate gland and is thus beyond the reach of local treatments such as surgery or radiation therapy. Hormonal therapy is also helpful in alleviating the painful and distressing symptoms of advanced disease. Further, it is being investigated as a way to arrest cancer before it has a chance to metastasize. Although hormonal therapy cannot cure, it will usually shrink or halt the advance of disease, often for years.

Surgery to remove the testicles (orchiectomy or surgical castration) is usually an outpatient procedure. The testicles are removed through a small incision in the scrotum; the scrotum itself is left intact. To help offset the operation's psychological toll, some men opt for reconstructive surgery in which the surgeon replaces the testicles with prostheses shaped like testicles.
