Primary Care

Primary care physicians, specialist physicians, hospital physicians, discharge planners, pharmacists, labs, imaging labs, living facilities and/or home health agencies have the best intentions but struggle to coordinate care. Additionally, the never-ending parade of bewildering medical bills often adds to a sense of overwhelming confusion and dismay.
Emergency Care
Your care team provides care in YOUR home – during our regular home visits we will evaluate your condition and provide hands-on care. Our goal is to help you avoid the exhausting effort required for Emergency Room visits and hospitalizations. There is a Registered Nurse available 24 hours each day, 7 days per week to answer YOUR questions, make visits and modify YOUR care when needed.

More than 1,800 veterans die every day – approximately 1 in 4 of America’s total deaths. We routinely provide care for a large number of veterans. Our care professionals have received specialized training in the areas of veterans’ needs specific to their culture, mental health issues and benefits as well as providing special recognition for veterans on our service. Our Volunteer Program also includes additional training for veteran services and we actively recruit veterans as hospice volunteers. Additionally, we provide outreach and education to veteran groups and community providers to increase awareness and earlier access to hospice care for veterans.
Behavioral Health
Under Centennial Care, each member must choose a MCO (Managed Care Organization/Insurance Company). Each member will have access to Care Coordination services and will receive a Health Risk Assessment (HRA) that will help members work together with their MCO to create a Care plan. This HRA is a telephone call from their MCO. If the HRA shows that the member has medical, behavioral health conditions or long-term-care needs that require a higher level of Care Coordination, the member will receive a Comprehensive Needs Assessment (CNA). The CNA will be completed in the members residence.

Ostomy Care – The management and support of a patient with a surgical opening created in the bladder, ileum, or colon for the temporary or permanent passage of urine or feces, necessitated by carcinoma, intestinal obstruction, trauma, or severe ulceration distal to the site of the incision. Patient will require replacement of ostomy bags and skin care several times per week.
