Hallmark Nursing Center, a Life Care Center of America facility is proud to offer a continuum of care with post-acute skilled nursing, short-term physical, occupational and speech therapies as well as outpatient rehabilitation. Rehabilitation is a focus at this facility, providing comprehensive individualized programs and services for patients of all ages. The therapy team communicates closely with the physician, nurses and social services, dietary and activities personnel to provide comprehensive care.
