Please read the following checklist carefully to be sure Rocky Mountain Health Centers Pediatrics is the appropriate medical home for your children. We look forward to meeting you and forming a mutually rewarding relationship.
Primary Care
Kaiser Medicaid As a policy holder of healthcare insurance it is your responsibility to be an informed consumer and inform us of any changes in your insurance coverage. If we are not provided with accurate and updated information, you may be responsible for payment in full for all services rendered. Pryor to your visit, it is your responsibility to verify our practice has a contract with your insurance carrier and that our physicians participate with your plan. It is expected that you have an understanding of what your policy covers, know your copayment amounts, deductible and coinsurance amounts. If your insurance carrier requires you to select a Primary Care Physician (PCP), it is also your responsibility to select our office prior to your visit. Any financial portion that is the “member’s responsibility” such as co-pay, deductible or a non-covered percentage will be collected at the time of service.

Ear Infections
Colds, upper respiratory infections, and URIs are common terms we use to describe viral illnesses that cause nasal congestion, runny nose, sneezing, sore throat, fever, and cough. The fever usually lasts for 2-3 days, and the cough with congestion and runny nose may last for 5-10 days. The typical preschool-age child may experience 6-10 colds per year. Most colds resolve on their own with rest and fluids, but some may lead to ear infection, sinus infection, asthma attack, or other complications. If you are concerned about the possibility of one of these complications, please have your child seen in our office for an evaluation.
Emergency Care
In the case of minor emergencies (lacerations, cuts, mild fractures, sprains), please call our office first for instructions because in some cases it might still be advisable for your child to be evaluated in an emergency department.

Please call the office soon to schedule your child's annual physical. Please remember we process MANY physical forms, immunization records, and medication permission forms (for administration of medication at school/daycare), so allow at least 3 business days for us to complete these forms.
