The employee is to report to Company Care one half hour prior to his/her appointment time and review with the clinicians personal and family history and the job description. The employee will undergo height, weight, pulse, respirations, temperature and blood pressure. Tests range of motion and physical examination by a physician designee can be provided. Additional testing for specific job needs are available for an additional fee. If the employee exhibits symptoms of a major medical or emotional problem, the employee will be advised to seek his/her own health care provider or be referred to an appropriate primary care giver. The examining physician/designee makes the final determination of an employee's ability to perform in a specific job. If the employee cannot perform in a specific job the employer will discuss this with the job candidate and determine if reasonable accommodations are available. If the individual refuses to comply with the practices of the preplacement physical examination process, she/he will be referred back to the Personnel Office of the employer.

These vaccines are recommended to protect travelers from illnesses present in other parts of the world and to prevent the importation of infectious diseases across international borders. The vaccination you need depends on your destination, whether you will be spending time in rural areas, the season of the year, your age, health status, and previous immunization history.
Electrocardiogram
The fireman exam to be performed on all appropriate candidates for the Fire Fighters positions will include: Complete medical history, Complete occupational history, Vital signs to include height and weight, temperature, pulse, blood pressure and respirations, Physical exam by physician, System review to include dermatological, EENMT, cardiovascular respiratory, GI, GU, Endocrine and metabolic, neurological, MS, Rectal exam with stool guaiac, Vision exam (peripheral, color, visual acuity), Pulmonary Function Test by a NIOSH Certified Clinician, Electrocardiogram, Audiology Exam by a CAQH Certified Clinician, Chest x-ray, Laboratory Studies (Complete Blood Count, Chemistry Panel, Glucose, Creatine, Lipid Panel, liver functions, Electrolytes, Urinalysis), and Musculoskeletal evaluation of the ability to perform the essential functions of the position.

When you get services from an in-network hospital or ambulatory surgical center, certain providers there may be out-of-network. In these cases, the most those providers can bill you is your plan's in-network cost-sharing amount. All employees using a negative or positive pressure tight-fitting face piece respirator must pass an appropriate qualitative fit test (QLFT) or quantitative fit test (QNFT). Fit testing is required prior to initial use, whenever a different respirator face piece is used, and at least annually thereafter. An additional fit test is required whenever the employee reports, or the employer or PLHCP makes visual observations of, changes in the employee's physical condition that could affect respirator fit (e.g., facial scarring, dental changes, cosmetic surgery, or an obvious change in body weight).
